Using Suffixes: Junior is preceded by a comma; when spelled in full, it is not capitalized.

Junior is preceded by a comma; when abbreviated, it is capitalized.

Professional Titles (guests): Professional titles are listed first, including cases where only the woman has the professional title.

If both doctors have separate last names, list them alphabetically and use separate lines.
